DC Residents

How to Apply for a Medical Marijuana Card in DC
  1. Schedule a visit with us HERE to receive an evaluation for recommendation.

    • You will need to get copies of your medical records from your primary care doctor that shows that you do indeed have a qualifying condition for this treatment. Please upload on our Scheduling page or fax to 877-560-3792. This must be either sent to us or brought in by you when you have your evaluation exam or this will delay the process. 

    • Upon determination of a qualifying condition, we will contact DC Department of Health and notify them of our recommendation of your qualification for medical marijuana. You can then apply for a state-issued Medical Marijuana card and this can then be taken to a dispensary where you can purchase your medical marijuana.

  2. Go to this website and apply to be a medical marijuana patient:

    • DC DOH Medical Marijuana Application

    • A fee of $100 is required for processing with an option for a reduced cost of $25 for those who qualify. See instructions HERE. Your Patient ID card will be mailed to you.

  3. For re-certification, you will need to schedule a visit (telemedicine option available). Aim to schedule and apply at minimum 6 weeks prior to expiration to ensure no lapse in treatment. 

MD Residents

How to Apply for a Medical Marijuana Card in MD
  1. Go to this website and apply to be a medical marijuana patient:

  2. You will need to get copies of your medical records from your primary care doctor that shows that you do indeed have a qualifying condition for this treatment. Please upload on our Scheduling page 

  3. Schedule an appointment for an initial evaluation HERE. This will include a review of your medical records and a brief physical exam. The cost for this evaluation will be $200 and will be collected at the time of the evaluation. This evaluation is not covered by insurance.

  4. Once we have determined that you qualify for medical marijuana, we will contact the Maryland Medical Marijuana Commission and notify them of our recommendation of your qualification for medical marijuana. However, not required, you can then apply for a state-issued Medical Marijuana card for a fee of $50. The Patient ID card will be mailed to you and can then be taken to a dispensary where you can purchase your medical marijuana.
